Some volunteer positions, especially those involving vulnerable populations (children, elderly, disabled), may require background checks. This form authorizes the organization to conduct such screening.<\/p>\n

    Part B: Background Check Authorization<\/h2>\n

    1. PURPOSE AND SCOPE<\/h3>\n

    1.1<\/strong>\u00a0I understand that the Organization I am applying to volunteer with may require a background check as part of their volunteer screening process. This is particularly important for positions involving\u00a0:<\/p>\n

      Handling of funds or financial information<\/p>\n<\/li>\n

      Positions of trust within the organization<\/p>\n<\/li>\n<\/ul>\n

      1.2<\/strong>\u00a0This authorization applies to the volunteer position for which I am applying and may be used for periodic checks during my volunteer service\u00a0.<\/p>\n

      3. CONSENT AND DISCLOSURE<\/h3>\n

      3.1<\/strong>\u00a0Voluntary Consent<\/strong>: I understand that my consent is voluntary. However, if I do not consent, I may be ineligible for certain volunteer positions\u00a0.<\/p>\n

      3.2<\/strong>\u00a0Scope<\/strong>: This authorization applies to the initial volunteer application and any periodic checks during my volunteer service\u00a0.<\/p>\n

      3.3<\/strong>\u00a0Information Accuracy<\/strong>: I certify that the information I have provided for identification purposes is true and complete.<\/p>\n

      4. USE OF INFORMATION<\/h3>\n

      4.1<\/strong>\u00a0Confidentiality<\/strong>: Any information obtained through background checks will be kept confidential and used solely for volunteer screening purposes.<\/p>\n

      4.2<\/strong>\u00a0Limitations<\/strong>: Information will not be used for any purpose other than evaluating my suitability for volunteer positions.<\/p>\n

      4.3<\/strong>\u00a0Retention<\/strong>: Background check records will be maintained in accordance with applicable laws and organizational policies.<\/p>\n

      5. RIGHTS UNDER FAIR CREDIT REPORTING ACT (FCRA) – US APPLICANTS<\/h3>\n

      5.1<\/strong>\u00a0If applicable, I understand that under the Fair Credit Reporting Act, if any adverse action is taken based on information contained in a consumer report, I have the right to\u00a0:<\/p>\n
