1. PURPOSE AND SCOPE #
1.1 I understand that the Organization I am applying to volunteer with may require a background check as part of their volunteer screening process. This is particularly important for positions involving :

1.2 This authorization applies to the volunteer position for which I am applying and may be used for periodic checks during my volunteer service .
2. AUTHORIZATION FOR BACKGROUND CHECK #
2.1 I hereby authorize and give the Organization permission to conduct a background check on me, which may include :

3. CONSENT AND DISCLOSURE #
3.1 Voluntary Consent: I understand that my consent is voluntary. However, if I do not consent, I may be ineligible for certain volunteer positions .
3.2 Scope: This authorization applies to the initial volunteer application and any periodic checks during my volunteer service .
3.3 Information Accuracy: I certify that the information I have provided for identification purposes is true and complete.
4. USE OF INFORMATION #
4.1 Confidentiality: Any information obtained through background checks will be kept confidential and used solely for volunteer screening purposes.
4.2 Limitations: Information will not be used for any purpose other than evaluating my suitability for volunteer positions.
4.3 Retention: Background check records will be maintained in accordance with applicable laws and organizational policies.
5. RIGHTS UNDER FAIR CREDIT REPORTING ACT (FCRA) – US APPLICANTS #
5.1 If applicable, I understand that under the Fair Credit Reporting Act, if any adverse action is taken based on information contained in a consumer report, I have the right to :

Dispute the accuracy or completeness of any information with the reporting agency
6. NOTICE FOR NEW YORK APPLICANTS #
6.1 For applicants in New York State, I acknowledge receipt of a copy of Article 23-A of the New York Correction Law, which governs the consideration of criminal history in screening decisions .
7. Indemnification #
7.1 I agree to indemnify and hold harmless the Organization from any claims arising from inaccurate information provided by me.
8. ACKNOWLEDGMENT #
8.1 I acknowledge that I have read and understood this Background Check Authorization and Consent Form.
8.2 I voluntarily agree to the terms and conditions stated herein.
